I cut the first part on my 5 axis mill. I helical-milled two holes and then with the tool tipped at 45 degrees, countersunk them using the side of the same end mill. Neither hole was at the center of the rotary table. The countersink cut was done by just commanding a C axis rotation. See the attached gcode. John K suggested this test part but as soon as he saw the photo, he suggested something much more complicated!
